Office Information: About Your Visit

Insurance Information

It is the new patient's responsibility to bring in their insurance information, ID, referral (if necessary) and Workman's Compensation information (if applicable). If you are unsure if a referral is necessary, please contact your insurance company via phone or internet to find out. Due to the number of insurance companies we work with, we cannot always tell you if one is required for your insurance company/plan.

Methods of Payment

We accept cash, checks, credit cards, and money orders.

Prescriptions & Renewals

Existing patients in need of a prescription refills, please call your pharmacy to request the prescription. They will fax us a request with all the needed information on it. Those needing a prescription for the first time from our office, please call our main office number 505-843-7798.

Medical Records

All new patients are responsible for collecting and bringing in all medical records that are relevant to what you will be seen for, or ensuring that the documents / films are at our office before their appointment. All returning patients are responsible for attaining MRI / CT / Bone Scan (etc.) films or ensuring they have arrived at our office before their appointment.
